A commercial flight attempting to land at a major urban airport in Brazil ended in one of the deadliest aviation accidents in the region’s history when the aircraft was unable to stop after touchdown and overran the runway.

The investigation revealed that the aircraft itself did not suffer a critical mechanical failure. Instead, a combination of challenging landing conditions and operational mistakes played a central role. The runway surface, known for its difficulty in wet conditions, provided reduced braking effectiveness and contributed to the aircraft’s inability to slow down in time.

During the landing sequence, confusion in the cockpit led to a critical error in thrust management. While attempting to decelerate, one engine remained at high power, preventing the aircraft from properly slowing despite braking efforts. At the same time, a lack of effective warning feedback meant the crew did not immediately recognize the configuration problem.

The aircraft ultimately overran the runway at high speed, resulting in a catastrophic impact in the surrounding area. The investigation concluded that pilot actions under pressure, combined with hazardous runway conditions, created a fatal chain of events.

In response, authorities introduced safety improvements to the airport, including runway modifications and operational restrictions during adverse weather. The tragedy remains a key case study in how environmental risk factors and cockpit decision-making can combine with devastating consequences.
